All of the forms needed for the certifications administered
or overseen by the Certification Board are located on the left.
Presently there
are 13 Initial Applications for
those of you seeking a Certification for the first time. There
are 11 Recertification
Applications, with 3 more being developed.
There is 1 Ethical Complaint form,
as well as 1 Request for Records
Verification Form.
In the Testing Forms section
there are 3 forms along with a calendar of Testing Dates. There
a 7 Education Manuals and
4 Miscellaneous Forms,
all of which should help you in your certification process.
Reciprocity: Anyone applying for Reciprocity, will now need to contact
the Certification Board for an application. Email us at info@certbd.com,
and you MUST include your Name and Address. It will be mailed to
you as soon as possible.
Important information regarding the LCADC/CADC.
The LCADC and CADC are under the authority of the Division of Consumer
Affairs, Department of Law and Public Safety.

All complaints, comments and questions regarding
the LCADC/CADC should go to the DCA, not the
Certification Board.
Note:
Initial Applications for certification need to be reviewed and the
Board needs time to perform its due-diligence duty, therefore we
have established cut-off dates for submission of Initial Applications,
and those dates are as follows:
- February 1 – for the March
Board Meeting
- May 1 – for the June Board Meeting
- August 1 – for the
September Board Meeting
- November 1 – for the December Board
Meeting
Important notice regarding receipt
of information: the Certification Board requires a “Return
Receipt” (the green post card from the Post Office) to be used
by the applicant to verify receipt of all information. The Board
will NOT respond to phone or email inquiries regarding
receipt of documents, because the card will have a signature and
the date the documents were received by the Certification Board.
