Note:
Initial Applications for certification need to be reviewed and the
Board needs time to perform its due-diligence duty, therefore we
have established cut-off dates for submission of Initial Applications,
and those dates are as follows:
- February 1 – for the March Board Meeting
- May 1 – for
the June Board Meeting
- August 1 – for the September Board Meeting
- November 1 – for
the December Board Meeting

Application is now available!
Fees:
We have increased our fees to cover increases in expenses and decreases in state aid. Our new fee schedule is:
- Initial Application: $225
- Recertification Application: $225
- Application Late Fee:$ 15 per month
We are working to update our forms, but applications will only be accepted with the above fees included as directed.
